To request your Provider password for the HealthEZ website, please enter your Federal tax id, City, State, Zip, and Email address below.
Once you are authenticated as a provider, your password will be emailed to you.
Avoid the use of your browser's navigation buttons as this can interrupt the flow of information from page to page.
